How Medicaid income limits work in 2026

Every state sets its Medicaid limits as a percentage of the federal poverty level (FPL), which HHS updates each January. The percentage depends on who is applying: adults, children, pregnant women, parents, seniors or people with disabilities. Because the FPL changes yearly and each state picks its own percentages, the dollar limit you need to compare your income against is different in every state and every year.

The two big divides

  • Expansion vs non-expansion. Forty states and DC cover all adults up to 138% FPL. The ten non-expansion states cover parents only, at limits as low as 16% FPL in Texas, and exclude childless adults entirely.
  • MAGI vs non-MAGI. Adults, children and pregnant women are judged on income only (MAGI rules). Seniors and people with disabilities also face an asset test, usually $2,000 for an individual, and different income rules.
