District of Columbia Medicare Savings Programs 2026: QMB, SLMB, QI Limits

Medicare Savings Programs (MSPs) use DC Medicaid funds to pay Medicare premiums and, for QMB, deductibles and copays. In 2026, a single person in District of Columbia can qualify for QI with income up to $3,990 a month, and for QMB up to $3,990.

A $20 general income disregard is applied before comparing to the limit in most states, so you can be slightly over and still qualify. Figures use 2026 poverty guidelines.

What each program covers

  • QMB is the most valuable: providers cannot bill you for Medicare deductibles or coinsurance, and the Part B premium is paid for you.
  • SLMB and QI pay only the Part B premium (about $185/month in 2025), which is added back to your Social Security check. QI is funded annually and served first-come, first-served.
  • All three automatically qualify you for Extra Help with Part D drug costs.

How to apply

Apply through the DC Department of Health Care Finance, not through Medicare. You can apply at the same time as full Medicaid; if you are over the Medicaid limit but under the MSP limit you get the MSP only.
